It can happen anywhere there are humans. I caught the local Chrysler dealership where I used to live charging my (at the time, elderly, now deceased) mother for the very first oil and filter change on the brand new Concorde she had bought - and they didn't change either. Before she took it in, I had checked the filter (didn't need to mark it - it was painted flat black and had a paint stamping on it saying "ORIGINAL FACTORY FILTER") and noted the color and level of the oil. When it came back, the same filter was still there, and the oil was the exact same level and color.
