Transmission Clunk, stuck in second gear limp mode?? HELP!?!?!

Hey all! looking for any info on where to go on my 2004 Intrepid ES 3.5l, she has about 100,000 something miles on her I just broke 100,000 about 3 days ago!!! about a month or so ago maybe longer she started doing a harsh clunk and then I find myself gear locked in second gear averaging around 40 at 3000 rpm and 50 at 4000 if I really push my line, I have read a few posts on here regarding the same problem and a lot of people have varying answers.

I personally have nailed it down to having a limp mode issue but the real question I have to anyone here is did replacing input and output sensors on the transmission solve the problem?
if so does anyone know where I can buy them?
I plan on doing as much as I can myself to the car to fix the problem before I pay someone else to figure out my problems since most transmission places want to play games and charge you money for things you don't need. I have also read that a solenoid can cause this issue as well, she has 100,047 miles on her at the moment, the issues started around 99,000, I am not 100% sure on the maintenance on the trans if any maintenance was done at all I am the 3rd owner with really no way to ask for records on when that kinda stuff was done. I know she is due for a complete flush and filter change on the trans, my check engine light is on as well, but I have always had the light on ever since I got the car with 96,696 on her 2 years ago.

I just need some fingers to point me in the right direction to help me save my baby, I have been looking at new rides but I kinda don't want to give up on my baby yet!! I recently had my oil pressure sensor switch go bad as well but I just replaced that thinking maybe there was a possibility it was triggering the computer to think something was off to trigger the trans. I also should mention that my PCM was reflashed back in 2007 by the second owner and I have no clue why or what the reasoning was behind the reflash, I kinda bought her without asking a ton of questions regarding things of that nature. I also have no clue of codes thrown yet regarding trans the only codes I have ever scanned for were for a hard starting problem I have had since I have owned the car as well, a buddy of mine has a scanner as well I can get newer codes if someone would need those to help determine my problem! I also have no motor issues other then the lack of power in limp mode.. :( Again any help is greatly appreciated!

Replacing the input and output sensors will not help with your model year.
(That will usually work for the 1998 - 2001 model years)

You will most likely need to drop the valve body and replace the solenoid pack.
While you're in there you can also install a Solenoid Switch Valve Plug from Sonnax.
(available from RockAuto for $15)

When you drop the valve body you can check the date stamped on the solenoid pack.
I'll bet the date stamp falls within the range noted below ( 3001 - 0603).

**If replacing the valve body assembly for DTC P1776 Solenoid Switch Valve Latched in
LR Position first inspect the build date on the solenoid pack (Fig. 1) or (Fig. 2). If the build
date range falls in between 3001 and 0603 replace the solenoid pack not the valve body**.

NOTE: The first three characters are the day of the year, the fourth character is the
year, ie. 3001 is the 300th day of 2001.
